All amounts listed are exclusive of GST.

Chronic Disease Management Plan (for Medicare rebates)
Private Health Insurance
Self-funded

If you have Private Health coverage, please contact your health fund to discuss possible rebates.

Please note: I do not bulk-bill and families are required to claim all rebates either online or in-person. Thank you for your understanding.
